There are two forms below, Form A and Form B. Form A is used when you place a Display ad image on the pages named above. These ads automatically include a FREE basic listing in one "Suppliers and Vendors" category of your choice.

Form B is used if you only want to place a listing in one or more of our "Suppliers and Vendors" categories, but you do NOT want to place a display ad.

Leave any fields blank for services you are not ordering. If you wish to place your display ad on both sides of the pages involved, or be in more than one "Suppliers and Vendors" Category, you may select multiples by holding down the "Ctrl" key. Be sure to let us know -- using the Upload page email form -- which type of listing should be placed in each category.

Please be sure you've viewed and printed the "Terms and Conditions" (PDF) document for your records, and for correct sizing of your ads/listing copy, as well as a complete price list.
